Completion of Medical Reports/Forms

Private Medical reports, completion of forms or GP letters for special purposes such as pre-employment, fitness to travel, elderly drivers, fitness to undertake sports or Power of Attorney etc do not form part of NHS Services and as such there may be a charge.

Please check the fee chargeable with a member of staff when you require to arrange any such report, form or letter.

Please allow at least 5 working days to receive your completed letter/form.
